About Florida Care Management

Florida Care Management was started by Sheryl Cohen in order to offer quality, credentialed geriatric care management services in Lady Lake, Fl. and surrounding area.  It is a fee for service, private pay agency.  Sheryl Bloom Cohen has 24 years of geriatric, older adult experience. 

Sheryl Cohen has a:

• BS degree in Sociology from Sacred Heart University

• MS in Education from Western Connecticut State University

• Masters in Social Work from the University of Cincinnati. 

Sheryl Cohen is a Licensed Social Worker and...

Carries NASW (National Association of Social Workers) certifications as an:

• Advanced Social Worker Care Manager

• Advanced Social Worker in Gerontology. 

Sheryl Cohen has worked both in long term care and as the Geriatric Social Worker at JFS, Cincinnati, OH where she developed their private care management practice. 

Sheryl has also taught courses on developing care plans at the University level, and has presented several times at the National Conference on Aging – American Society on Aging National Conferences.  She was a board member, for 6 years, on The Association for Professionals on Aging, Cincinnati, Ohio Chapter. 

• She was on the planning committee for the Midwest regional GCM conference. 

• She has been a field supervisor for graduate level MSW interns at the University of Cincinnati.
